Please note you will need the following documents to enroll a student at WSHS:
· Birth Certificate
· Proof of Authorized Person to Enroll Student
*ID (If not the parent, they must bring in Caregivers Affidavit from Child Welfare and Attendance Office)
· Immunization Records
· Transcript from the previous school
· Proof of Address such as a utility bill or lease agreement

· Acta de Nacimiento
· Prueba de persona autorizada para inscribirse: identificación (Si el estudiante no vive con los padres- se require uno de los siguientes:
*Declaración Jurada Para Autorización de Tutor de la Oficina de Bienestar Y Asistencia.
· Cartilla de Vacunación
· Historial Académico de calificaciones de la escuela anterior
· Comprobante de domicilio como factura de teléfono, cable, electricidad, gas o contrato de arrendamiento

Transcripts prepared by the registrar will be mailed via the U.S. postal service to the colleges, universities, or scholarships of your choice.
Transcripts prepared by the registrar may not be tracked and the school cannot accept responsibility for documents sent by the U.S. Postal Service.
