If your child is going to be participating in sports at West Shores they must visit the Athletic Clearance website. On this website you will find our athletic package that must be completed prior to attending practice.

If your child is under the age of 14 he/she will not be allowed to participate in our football program. Once your child reaches the age of 14, a consent form must be submitted along with his/her physical.

Please print out the form and return it to the Athletic Director.
